GP Chronic Condition Management Plans
Medicare referrals under a GP Chronic Condition Management Plan (GPCCMP) are accepted at both of our Biotune Chiropractic locations.
What is a GP Chronic Condition Management Plan (GPCCMP)?
GP Chronic Condition Management Plans (GPCCMP) were previously known as GP Management Plans (GPMP), Team Care Arrangements (TCA) Plans, and Enhanced Primary Care (EPC) Plans. This care plan is prepared by the client’s General Practitioner for individuals with a chronic or long-term health condition lasting six months or more.
The plan is designed to support coordinated care. It may outline the client’s health needs and agreed care goals, detail the care provided by the GP, and coordinate referrals to eligible allied health professionals where appropriate.
Key Information About the Plan
- The care plan must be prepared and issued by a GP
- Medicare provides rebates for up to five allied health services per calendar year
- The calendar year runs from 1 January to 31 December
- Services may be shared across multiple allied health providers or used with a single provider
- Any unused services can carry over into the next calendar year, but will count towards that year’s total of five Medicare-subsidised allied health visits
Referral Guidelines and Validity
GPCCMP referrals are valid for 18 months from the date of issue, unless all approved services are used before that date. Referrals apply only to the number of chiropractic services specified by the GP. A new referral is required once all referred services have been used, if the referral has expired, or if a referral to a different allied health service is needed.
Our Fees at Biotune Chiropractic
While Medicare provides a rebate for GPCCMP services, a small gap fee applies at Biotune Chiropractic. The Initial Consultation and Case Review is $32, and a Standard Consultation has a $20 gap, payable by the client on the day of the appointment.
