The paperwork you’ll need to fill out for your first visit can be completed at the practice, please arrive 5 minutes before hand. Please wear comfortable clothing and bring along any x-rays or additional imaging you may have.

Getting to the Cause
When you arrive to your initial consultation appointment, one of our chiropractors will sit down with you one on one to take a full history, allowing us to understand what has led you to seek our help and what you would like to achieve. Once we have an idea of what needs to be improved, we will complete a physical examination with specific Orthopedic & Neurological testing to pinpoint the source of your problem to determine if you are a chiropractic case. This visit includes treatment, (if clinically safe) and takes about 30 minutes to complete.
Report Visit
Your second visit (the Report of Findings) includes a verbal report where we will explain our findings and make recommendations best suited to your needs so you can make an informed decision about the best way to move forward with your treatment plan.
